We see Mal and Cobb awake just once - when they're in the room and Cobb hugs her - yet Cobb said they were experimenting with 'dreams within dreams' - we never see them wake up 'again' - does that mean Mal was right and they were still in the first dream, so her jumping from the ledge would have taken her back to reality?

(1)See the answer of "What is limbo?":
If one dies in limbo, one would wake up in reality.
or..
(2)The movie just didn't show any subsequence suicides. The "inception" Cobb planted in Mal was too powerful she will never accept that she was in reality. The two woke from one layer at a time until the last one where she jumped from the ledge, in the world which Cobb believed it was in reality.
